If a scenario doesn't call for a formation's chit to be in the initial mix, is it specifically excluded from the scenario, or can it be purchased
during a game turn?

They can be purchased later on, providing the leader is on the map (or in the case of the independent formation activation chit, there is an independent Unit
on the map).

If a new formation is in a reinforcement hex, preparing to enter on the current turn, is it considered "on the map" for the purpose of adding its
formation chit for that (or the next) turn?

For reinforcements, the formation activation chit becomes available for purchase the turn after the leader enters the map (i.e. it leaves the reinforcement
hex). See the second to last paragraph of Series rule 7.2.
